During your initial and follow up appointments, I will repeatedly evaluate your need to be placed on controlled substances. While Benzos, Hypnotics and Stimulants are controlled for a reason, they are also excellent short term medications. They have been designed and marketed for short term usage. Long term usage will lead to sequelae and potential undesired side effects. I believe in the optimal use of these medications, it is important that they are consumed as prescribed. → Cancellation/No Show Policy

Life happens. We understand. If you cancel 48 business hours ahead, there is no cancellation fee. Between 48 business hours to 24 business hours - we will charge a 50 % fees. Cancellations done within 24 business hours, APA guideliness stand by the provider and suggest that the provider is fully protected for charging the fees. This is because when we book appointments, we invest in your mental health. It is only fair that we get compensated for our time and expertise.

→ Doctors Note, Emotional Support Animal note & FMLA

A doctors note will be provided upon request, after every appointment. Emotional Support Animal[ESA] letter and documents - we are happy to provide an Aviation approved format for ESA, if you qualify. All the other documents need to be procured by you. FMLA is an entirely different beast. It needs the beneficiary to be frequently assessed and repeatedly evaluated. This is best done by your primary care physician both from a financial perspective as well as timeline. However, if you wish to have it done by us, we will evaluate it on a case by case basis and it is solely at the discretion of the provider.
